If you were a previous existing patient with Dr. Jazmine, but have not been seen by her in the last six months, a new patient examination and paperwork is required for their first appointment.

If you do not see an available appointment time on the day you need an appointment, please text or call 336-303-8377 to see if a cancellation or reschedule opened a spot for you.

Please ensure you fill out the appropriate paperwork if you are a new patient.
